On or about October 9, 2019, the employer failed to provide and require the use of protective clothing for the thermal hazards related to oxygen fire and explosion risk associated with transfilling oxygen cylinders tasks. Aircraft service employees utilized a compressed gas system that included a Futurecraft oxygen intensifier without PPE for protection against thermal hazards. On September 17, 2019, an employee suffered serious injuries that included burns, from an oxygen explosion/fire event related to required oxygen transfilling duties.
