29 CFR 1910.269(a)(2)(v)(A): Retraining was not conducted whenever a periodic inspection under paragraph (d)(2)(v) of this section reveals, or whenever the employer has reason to believe, that there are deviations from or inadequacies in an employee's knowledge or use of the energy control procedures. At the facility located at 12567 FM 3430 Vernon, Texas: The employer did not ensure employees retained the knowledge and skills necessary for the control of hazardous energy and the use of personal protective equipment. An employee trouble shooting Transformer Rectifier (TR) equipment without tagging out and not using personal protection equipment was exposed to arc flash burns. The employer is to provide retraining to exposed employee's.
