Text For Citation: 01 Item/Group: 004A Hazard:
29 CFR 1910.303(b)(2): Listed or labeled electrical equipment was not used or installed in accordance with instructions included in the listing or labeling: a) Building #672, Range Masters Office: Two relocatable power taps; a multi receptacle extension cord; and a duplex receptacle flat extension cord were series connected (daisy chained) to provide power from a wall mounted electrical outlet to a piece of electrical equipment from one side of the room to another. b) Building #672, Break Area: A 120 volt ISOBAR Model 1B-4 relocatable power tap designed and listed for high concentrations of low powered loads was instead used to provide power to high powered loads such as; An Emerson Microwave Oven Model MW119W; A Black and Decker Toaster Oven; and a GE full sized refrigerator. c) Building #26, South wall of the Break Room: A Surge Guide Relocatable Power Tap violated it's listing by (a) being screwed into the masonry wall rendering it un-relocatable; and (b) Providing power to high powered loads such as: A Black and Decker Toaster Oven and a; GE Microwave Oven. d) Building #26, West wall of the Break Room: A 120 volt Relocatable Power Tap violated it's listing by (a) being screwed into the masonry wall rendering it un-relocatable; and (b) Providing power to high powered loads such as: A Full sized Whirlpool Refrigerator and a; Hotpoint Microwave Oven. e) Northwest corner of Building #26: A Tripp-Lite Relocatable Power Tap providing power to a job made "sandwich/food" heater, was secured to a nearby metal fixed ladder with plastic ties, violating it's listing by being un-relocatable. f) Southeast Corner of the Paint Locker: A 120 volt New Point Relocatable Power Tap violated it's listing by being zip tied to a wooden post rendering it un-relocatable. g) Northwest corner of Building #26 and the Paint Locker: The following cords were daisy chained (series connected); A Tripp-Lite relocatable power tap was plugged into a wall mounted receptacle outlet. A 100 foot 120 extension cord was plugged into that power tap, where it ran outside and into the Paint Shed were a New Point relocatable power tap was plugged into it. ABATEMENT CERTIFICATION IS REQUIRED
