29 CFR 1910.28(b)(11)(ii): Each flight of stairs having at least 3 treads and at least 4 risers was not equipped with stair rail systems and handrails in accordance with Table D-2, Stairway Handrail Requirements: The employer does not ensure that each flight of stairs having at least 3 treads and at least 4 risers are equipped with stair rails. a) This violation was observed on or about May 30, 2019, at the Diamond Back entrance for maintenance, stairs with 4 risers were not provided with stair rails, exposing employees to fall hazards. b) This violation was observed on or about May 31, 2019, in Fort Frontier, stairs with six risers were not provided with stair rails, exposing employees to fall hazards. c) This violation was observed on or about June 3, 2019, in the north games storage area where on the second ninety degree turn on stairs leading to the second level storage area were not provided with handrails, exposing employees to fall hazards. Pursuant to 29 CFR 1903.19, within (10) calendar days of the date of this citation, the employer must submit documentation describing the steps taken to ensure each flight of stairs having at least 3 treads and at least 4 risers are equipped with stair rails.
