29 CFR 1910.28(b)(9)(i)(A): Each fixed ladder installed before November 19, 2018 was not equipped with a personal fall arrest system (PFAS), ladder safety system, cage, or well: The employer does not protect employees from fall hazards. At 10047 Compton Street, Belgrade, Missouri, specifically at the Feed Mill, employee(s) would climb up four, fixed ladders to the top of four, grain bin silos to check on the level of commodity inside. Each ladder was not equipped with a PFAS, ladder safety system cage, or well. The fall distance was approximately 25 feet to the concrete below. This violation was noted on 05/22/19.
