29 CFR 1926.451(a)(6): Scaffolds were not designed by a qualified person and were not constructed and loaded in accordance with that design. (General Industry reference 29 CFR 1910.27(a)) On or about March 19, 2019, and times prior thereto, the employer allowed employees, who were engage in welding work activities, to work from a mast-climbing work platform that was not designed and constructed with that design by a qualified person. This exposed employees to caught between and/or fall hazards.
