29 CFR 1910.212(a)(3)(ii): Point(s) of operation of machinery were not guarded to prevent employee(s) from having any part of their body in the danger zone(s) during operating cycle(s): Location: Facility On or about January 10, 2019, employees were not protected from point of operation hazards and exposed to laceration and amputation hazards while operating the following unguarded and inadequately guarded alligator shears and compactor; 1. Aluminum processing area: Gensco alligator shear had no guard to protect the operator from the point of operation. 2 and 3. Copper processing area: Two Gensco alligator shears, model #T4 - guards were inadequate in that they manually swing up and out of the way and does not protect employees from the point of operation. 4. Copper processing area: Constellation, model #1.5, copper alligator shear had no guard to protect the operator from the point of operation. 5. Copper processing area: Piqua compactor's steel mesh door guard was damaged and not operable and the safety interlock device was bypassed and did not protect employees from the point of operation.
