29 CFR 1910.119(l)(4): If a change covered by this paragraph results in a change in the process safety information required by paragraph (d) of this section, such information was not updated accordingly. On or about 4 September 2018, and at times prior thereto, the employer did not ensure the process safety information was updated accordingly from a management of change on an ammonia covered process equipment. The system was upgraded from a 14-ton IMECO Air Handling Unit to a 17-ton Krack Air Handling Unit without updating the Piping & Instrumentation Diagrams (P&ID's).
