29 CFR 1926.451(g)(4)(i): Guardrail systems were not installed along all open sides and ends of platforms. a) Jobsite: 3525 I Street, Philadelphia, PA 19134 building Rooftop - Employees conducting masonry work from a two point suspended scaffold on a 5th story window were exposed to fall hazards. A complete guardrail system consisting of either midrails, screens, mesh, or other intermediate vertical member no more than 19" apart was not provided between the top railing and the scaffold platform. This was documented to occur on August 8, 2018.
