29 CFR 1926.454(a): The employer did not have each employee who performed work while on a scaffold trained by a person qualified in the subject matter to recognize the hazards associated with the type of scaffold being used and to understand the procedures to control or minimize those hazards as required by paragraph (a)(1)-(5) of this section: The employer is failing to ensure employees who are working on tubular frame scaffold platforms are trained by a person qualified in the subject matter to recognize the hazards associated with the type of scaffold being used. This was most recently documented at 4708 S.48th St. Lincoln, Nebraska on or about July 5, 2018. Employees were working on a platform elevated approximately 14 feet which had multiple issues including but not limited to, partially planked working surface, not mudsills and no standard guard rail system.
