29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: A. Fiber Baler: On, or about, July 9, 2018, an employee suffered serious hand injuries while assisting with a manual baler wire insertion. The Harris HLO baler had not been turned off and locked out and the task required the employee to reach into a hazardous area of the machine. B. Conveyor 81A: On, or about, July 10, 2018, an employee entered the screener section of the machine for daily cleaning. The machine was not locked out at the main disconnect in accordance with the established procedure, instead one of the three shaft motors was used. Also, the machine controls were not operated to ensure that the conveyor shafts were properly locked out.
