29 CFR 1926.502(d)(15)(ii): Anchorages used for attachment of personal fall arrest systems were not designed, installed and used under the supervision of a qualified person: The employer is failing to protect employees from a fall hazard. Two World Wrecking employees were using a single-point adjustable suspension scaffold to access an empty elevator shaft and were not protected by an adequate personal fall arrest system. Employees were not attached to independent vertical lifelines with approved anchorages.
