29 CFR 1910.215(a)(4): Work rest(s) on grinding machinery were not adjusted closely to the wheel with a maximum opening of one eighth inch: On or about 5/22/2018, at the maintenance department of CGI Windows and Doors Inc., 3780 West 104th St, Hialeah, FL. An employee was exposed to a struck by hazard while working with the Palm Green bench grinder, Model No. 9682073, that had its right side workrest placed at 1/4-inch from the wheel.
