29 CFR 1910.28(b)(1)(i): The employer did not ensure that each employee on a walking-working surface with an unprotected side or edge that was 4 feet (1.2 m) or more above a lower level was protected from falling by one or more of the following: Guardrail systems, safety net systems, or personal fall arrest systems: a) On or about 4/17/2018, In the Cruciible Area: A platform was not provided with a guardrail system or other means of fall protection. Employees exposed to falls of 5.5 feet. b) On or about 4/17/2018, In the Melt Area: An opening at the ESR 2 furnace was not provided with guardrails or other means of fall protection. Employees exposed to falls of 18 feet. c) On or about 4/18/2018, In the Bar Mill: There was lack of a midrail near the stairs to the Orbit saw platform. Employees exposed to falls of 6 feet. d) On or about 4/18/2018, In the Bar Mill: The platform located behind Orbit Saw 1 was not provided with a midrail. Employees exposed to falls of 8 feet.
