29 CFR 1910.333(b)(2)(iv)(B): Before circuits or equipment was worked as deenergized, a qualified person did not use test equipment to test the circuit elements and electrical parts of equipment to which employees would be exposed. The qualified persons also did not determine if any energized condition existed as a result of inadvertently induced voltage or unrelated voltage backfeed even though specific parts of the circuit had been deenergized and presumed to be safe. 100 Simplex Dr, Westminster, MA On March 27, 2018 and at times prior, employees who maintained electrical equipment and components did not use electrical test equipment to ensure deenergization, the lack of induced voltage, and the lack of voltage backfeed.
