29 CFR 1915.12(a)(1)(ii): The employer did not ensure that spaces and adjacent spaces that contain or have contained combustible or flammable liquids or gases were visually inspected and tested by a competent person to determine the atmosphere's oxygen content prior to initial entry into the space by an employee. (a) William E. Strait - On or about January 19, 2018 the employer required employees to enter spaces to perform work such as but not limited to pulling electrical wire and did not ensure a competent person tested the atmosphere's of the space to determine the oxygen content prior to initial entry into the space by employees exposing employees to asphyxiation hazards.
