29 CFR 1926.1053(b)(1): Because of the portable ladder's length, the ladder side rails did not extend to at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access: a) Entire fourth floor, on or about 3-23-18 and 3-27-18: Employees were using step ladders to access roof trusses that were not 3 feet above the landing surface, in that the top of the ladders were 44 inches below the landing surface, exposing the employees to a fall hazard of 8 feet up to 11 feet 8 inches.
