Text For Citation: 01 Item/Group: 004A Hazard:
29 CFR 1926.1431(j)(1): Prior to hoisting employees on the personnel platform, and after any repair or modification, the platform and rigging was not proof tested to 125 percent of the platform's rated capacity: (a) On or about March 20, 2018, and at times prior, the employer did not ensure that prior to hoisting employees on the personnel platform, the platform and rigging was proof tested to 125 percent of the platform's rated capacity. Employees were exposed to fall hazards in excess of 50 feet while being hoisted on a basket drill to conduct rock fall mitigation activities. Basket drill DR-52 was not proof tested prior to hoisting employees. Abatement Note: Abatement certification is required for this item (see enclosed "Certification of Corrective Action Worksheet").
