29 CFR 1926.501(b)(4)(i): Each employee on walking/working surfaces was not protected from falling through holes (including skylights), more than six feet (1.8 m) above lower levels, by personal fall arrest systems, covers, or guardrail systems erected around such holes. a) Worksite was located at 235 Grand Street, Jersey City, NJ 07302 Employees were exposed to fall hazards while working in close proximity to open holes approximately twelve inches by eighteen inches in the flooring while decking the second floor southwest. The open holes were over uncapped vertical rebar on the next lower level approximately nine feet down. Violation was observed on or about December 11th, 2017. b) Worksite was located at 235 Grand Street, Jersey City, NJ 07302 Employees were exposed to fall hazards as there were open holes approximately two feet by two feet in the flooring where employees were doing layout on the seventh floor above a concrete floor on the next lower level approximately nine feet down. Violation was observed on or about December 11th, 2017.
