29 CFR 1910.147(c)(7)(i): The employer did not provide adequate training to ensure that the purpose and function of the energy control program was understood by employees: a) Mercy Health Systems, d.b.a. St Vincents Medical Center, located at 2213 Cherry Street in Toledo, Ohio; On or about November 6, 2017 the employer did not ensure employees were adequately trained in the purpose and function of the energy control program. Employees were required to perform activities such as, but not limited to, leaning into the San-I-Pak trash compactor to align pins. Employees entering (upper body) the San-I-Pak trash compactor, a decommissioned autoclave, were exposed to crush and amputation hazards from the energized ram of the compactor.
