29 CFR 1910.303(b)(1)(i): Electric equipment was not free from recognized hazards that were likely to cause death or serious physical harm to employees, in that, the employer's failure to consider the suitability of electrical connector installation exposed employees to a workplace crushing hazard: (a) L&R 25 Ton Over Head Crane (Serial No. 12745); West Bay South Corner: Electrical connectors replaced on the overhead crane and supplying power to the lifting magnets, were not verified as having a good connection before being placed back into service. This exposed employees to a crushing and struck-by hazard.
