29 CFR 1910.146(j)(2): The employer did not ensure that each entry supervisor verified, by checking that the appropriate entries had been made on the permit, that all tests specified by the permit had been conducted and that all procedures and equipment specified by the permit are in place before endorsing the permit and allowing entry to begin: a) In the North Shop, on or about July 20, 2017 - The employer allowed entry into the high pressure tank car TILX 303142 before atmospheric testing specified by the permit was conducted. A tank welder entered the tank to set up for a post weld heat test. b) In the North Shop, on or about July 20, 2017 - The employer allowed entry into the high pressure tank car TILX 303142 before car ventilation specified by the permit was conducted. A tank welder entered the tank to set up for a post weld heat test. c) In the North Shop, on or about July 20, 2017 - The employer allowed entry into the high pressure tank car TILX 303142 before verifying that an attached lifeline and harness specified by the permit was being used by a tank welder. A tank welder entered the tank to set up for a post weld heat test with a harness that was not attached to a lifeline.
