On or about May 31, 2017, at the Sumitomo SE180EV injection molding machine conveyor, the rotating unprotected keyway shaft was protruding out approximately 2.5 inches. Employees work and walk by the conveyor with the rotating protruding shaft. Employees exposed to the hazard of being caught-in the rotating protruding shaft.
