29 CFR 1910.242(a): Hand and portable powered tools or equipment were not kept in safe condition: (a) 101 North Main Street, Newark, Illinois, west building shop, DeWalt and Metabo portable handheld angle grinders: on or about May 4, 2017, employees using equipment for regular fabrication tasks were exposed to contact with live electrical parts likely to result in serious injury because flexible power cords were damaged exposing internal wiring. (b) 101 North Main Street, Newark, Illinois, west building shop, DeWalt and Metabo portable handheld angle grinders: on or about May 4, 2017, employees using equipment for regular fabrication tasks were exposed to contact with rotating grinding wheel hazard likely to result in serious injury because guards were missing.
