29 CFR 1910.132(a): Protective equipment, including personal protective equipment for eyes, face, head, and extremities, protective clothing, respiratory devices, and protective shields and barriers, was not provided, used, or maintained in a sanitary and reliable condition wherever it was necessary by reason of hazards of processes or environment, chemical hazards, radiological hazards, or mechanical irritants encountered in a manner capable of causing injury or impairment in the function of any part of the body through absorption, inhalation or physical contact. a. Ross Casting & Innovation, LLC located in Sidney, Ohio: On or about April 21, 2017, the employer did not ensure an employee working on the APF 5 was wearing clothing to protect the upper and lower body from incidental aluminum splashes. The employee was wearing a sleeveless cotton t-shirt and blue jeans when skimming dross and adding aluminum ingots to the molten aluminum furnace. b. Ross Casting & Innovation, LLC located in Sidney, Ohio: On or about May 25, 2017, the employer did not ensure an employee working on the APF 10 was wearing clothing to protect the upper and lower body from incidental aluminum splashes. The employee was wearing a cotton t-shirt and blue jeans when skimming dross and adding aluminum ingots to the molten aluminum furnace. c. Ross Casting & Innovation, LLC located in Sidney, Ohio: On or about April 21, 2017, the employer did not ensure an employee working on equipment such as but not limited to the APF 5 was wearing protective equipment to protect the employee's lower arms from hot surfaces created by the APF furnace and the core oven. The employee was wearing a sleeveless cotton t-shirt and cotton sleeves when working around the hot APF plate and the core oven drawers but was still sustaining serious burns from the hot surface.
