Located at 1678 Wrenbury Drive in Galena, Ohio: On or about March 31, 2017, an employee was bracing a steel I-beam from a six foot Werner self-supporting fiberglass ladder that was placed on an uneven gravel surface, thereby exposing the employee to a fall hazard.
