29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees are engaged in the activities covered by this section. On or about February 21, 2017, in the Oil Movement Process Area at Blanchard Refining Company, LLC located in Texas City, Texas; the employer did not utilize the procedures for the control of hazardous energy, including but not limited to specific lockout/tagout (LOTO) and line clearing procedures when the crude charge pump J27 was decoupled for motor rotation testing.
