29 CFR 1910.303(b)(1): Electrical equipment was not free from recognized hazards that were likely to cause death or serious physical harm to employees, in that the employer did not ensure the safety of the equipment pursuant to the considerations in sections (i) through (viii) of 29 CR 1910.303(b)(1): Location: a) Wall behind Mycom Compressor RC-6, Ammonia Engine Room, 500 Fenimore Road, Mamaroneck, NY 10543 b,c,d,e) Sump pump contactors 1&2, Ammonia Engine Room, 500 Fenimore Road, Mamaroneck, NY 10543 On or about 11/14/16 a) A section of the protective front cover on the 208/120 volt wireway was missing. b) The face plate of a 120-volt wet location outlet box was wrapped and held in place with electrical tape. c) Conductors entering a 120-volt male plug did not have strain relief. d) Conductors entering a 120-volt female plug did not have adequate strain relief. e) Conductors in a wet location were connected using a "wire nut" wrapped in tape that did not create a seal.
