29 CFR 1910.147(e)(3): Each lockout or tagout device was not removed from each energy isolating device by the employee who had applied the device: (a) On or about February 15, 2017 and at times prior thereto, the maintenance supervisor removed lockout devices placed on machines by maintenance employees, exposing employees to hazards associated with the unexpected release of hazardous energy.
