29 CFR 1910.147(d)(5)(ii): Where there was a possibility of re-accumulation of stored energy to a hazardous level, verification of isolation was not continued until the servicing or maintenance was completed, or until the possibility of such accumulation no longer existed. On September 7, 2016 and at times prior thereto in the Alky 3 Unit at the Marathon Galveston Bay Refinery where there was a possibility of re-accumulation of stored energy in the discharge piping from the P-1008B Depropanizer Reflux Pump, a segment of piping where a line opening was to occur, the employer did not continue to verify the isolation until the maintenance work was completed or possibility of such accumulation no longer existed.
