29 CFR 1926.1211(c)(1): The employer did not ensure that each authorized entrant must use a chest or full body harness, with a retrieval line attached at the center of the entrant's back near shoulder level, above the entrant's head, or at another point which the employer can establish presents a profile small enough for the successful removal of the entrant: a. E E Robinson Park (Level Creek), 800 Peachtree Industrial Blvd. Sugar Hill, GA - An employee was exposed to asphyxiation hazard. The employer did not ensure the entrant used a chest or full body harness, with a retrieval line attached at the center of the entrant's back. The employer removed the manhole cover and purchased a flash light with the expectation that the employee would break the plain, on or about September 6, 2016.
