29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: On or about August 25, 2016, and at time prior thereto, at this manufacturing plastic plant production area where employees were exposed to struck-by, caught-in, mechanical, thermal, stored, pressurize and electrical hazardous energies when performing service and/or maintenance, the employer control procedures had not been developed and utilized to control all hazardous energies for each type of equipment or machine: a. CNC Robot automatic system; b. Plastic molding press; c. Hydraulic injection molding machines MG-2300; d. Hydraulic injection molding machines MG-4400; e. Heat Controller Machine; f. Router 1; g. Dryer Hopper Temperature Controller #1; h. Dryer Hopper Temperature Controller #2; i. Dryer Hopper Temperature Controller #5; j. Carousel Plus Dehumidifying Dryer; k. Gravimetric Batch Blender Mixer; l. Direct Gas-Fired Industrial Air Handler; and, m. Rotary Screw Air Compressor.
