29 CFR 1926.451(g)(1)(vii): Each employee on a scaffold, not otherwise specified in paragraphs (g)(1)(i) through (g)(1)(vi) of this section, more than 10 feet (3.1 m) above lower level was not protected from falls by the use of personal fall arrest systems or guardrail systems meeting the requirements of paragraph (g)(4) of this section: a. At the east, south and west side of the south building, employees were exposed to a 21 foot fall hazard, in that, a tubular welded frame scaffold 26 frames wide and four frames high did not have a railing system to include a midrail on the second and third exterior frames of the scaffold, on or about 07/07/2016. b. At the east, south and west side of the south building, employees were exposed to a 27 foot fall hazard, in that, a tubular welded frame scaffold 26 frames wide and four frames high was not provided with railings at the end-platforms of the second, third and fourth frames, on or about 07/07/2016.
