29 CFR 1910.119(j)(2): The employer did not establish or implement written procedures to maintain the on-going integrity of process equipment: a) In the propane storage area at 106 Lonczak Drive Chicopee, MA: On or about April 15, 2016, the employer did not establish or implement written procedures to maintain the on-going integrity of the polyethylene piping between the propane storage tank and facility exterior. b) Between the facility exterior and the heat treat department at 106 Lonczak Drive Chicopee, MA: On or about April 15, 2016, the employer did not establish or implement written procedures to maintain the on-going integrity of the metallic piping supplying propane to the heat treat furnaces and the flexible hose in the propane distribution area for the two banks of rotary furnaces. c) The two rotary furnace pits in the heat treat department at 106 Lonczak Drive Chicopee, MA: On or about April 15, 2016, the employer did not establish or implement written procedures to maintain the on-going integrity of the Scott Instruments Sentinel II series 7200, gas detection systems in that the lower explosive limit (LEL) sensors had been installed for approximately ten years without being calibrated.
