29 CFR 1910.23(a)(5): Every pit and floor opening was not adequately guarded by standard railing or any other effected means: A.) Falls Creek Powdered Metals, Inc., 82 Industrial Drive, Brockway, PA, 15824, production area, on or about November 23, 2015 and times prior thereto: Employees walk/work in the area of the pit at the Alpha Press brand powered metal press (M8). The 29.75-inches pit was not completely guarded leaving several areas of the pit open including, but not limited to an area of 13-inches by 33-inches.
