NOTE: Proper application of the decision flow chart in Appendix A to section 1910.146 would facilitate compliance with this requirement. a. The employer had not finished evaluating the workplace for permit-required confined spaces, thereby exposing employees to atmospheric hazards such as but not limited to oxygen displacement and low oxygen levels.
