29 CFR 1910.147(c)(4)(ii)(B): The procedure did not clearly and specifically outline the specific procedural steps for shutting down, isolating, blocking and securing machines or equipment to control hazardous energy. On or about October 2, 2015, and times prior to, at the MEMC Pasadena, Inc. facility in Pasadena, TX, the employer did not ensure that specific steps for isolation, blocking and securing the K92350 Silane compressor was in place before employees began blinding the system for maintenance.
