29 CFR 1910.23(c)(1): Open-sided floors and/or platforms four feet or more above adjacent floor or ground level were not guarded with standard railings (or equivalent) and toeboards: A.) Warren Railcar Service, Inc. - On or about 8/19/15, and at times prior thereto, employees were walking on the top of railcars without fall protection. Work activities included, but were not limited to, welding work and inspection activities. Employees were exposed to a fall hazard of at least 14 feet. In the alternative: 29 CFR 1910.132 (a): Protective equipment was not provided, maintained, and used wherever it is necessary by reason of hazards: A.) Warren Railcar Service, Inc. - On or about 819/15, and at times prior thereto, employees were walking on top of railcars and were not wearing any personal fall arrest. Work activities included, but were not limited to, welding work and inspection activities. Employees were exposed to a fall hazard of at least 14 feet.
