29 CFR 1910.147(c)(7)(i)(A): Authorized employee(s) did not receive training in the recognition of applicable hazardous energy sources, the type and magnitude of the energy available in the workplace, and the methods and means necessary for energy isolation: The employer is failing to provide adequate lockout and tag out training to authorized employees. This was most recently evident during the inspection of the facility located at 2500 Old Hadar Rd & 2300 Riverside Blvd. Norfolk, NE 68701; where the employer has employees who operate and or perform maintenance on equipment that requires them to lock the machine out against hazardous energy sources through the use of lockout and tag out, they have been authorized by the company to perform lockout and/or tag out however they have not been given the required training. Instances include but are not limited to: i. Employee clearing jams out of machines in the Fiber Line, Thermal Pad Line and the Main Production Lines. ii. Employees cleaning the machines weekly where they are required to apply lockout and/or tag.
