29 CFR 1910.146(c)(1): The employer did not evaluate the workplace to determine if any spaces were permit-required confined spaces: Employees were exposed to potentially hazardous atmospheres within the 5-feet deep basement dirt crawl-spaces accessible through the trap door and ladder located inside the pantries of the Memory building (building 1) and the Assisted Living building (building 2) and the employer did not make a determination of the hazards in these spaces that contained the furnace and ducts.
