29 CFR 1910.304(g)(6)(vi)(C)(5): The employer did not ensure that exposed noncurrent-carrying metal parts of cord-and-plug connected equipment that may become energized was grounded when the equipment was used in damp or wet locations by employees standing on the ground. a) For employees working at 5127 Bloomingdale Avenue in Tampa, as observed on or about 24 September, 2014. Employees were using 2 wire non grounded handheld lamps. the lamp cord was repaired and the polarity was reversed. Employee was in the process of changing the light bulb inside of one of the metal archways when contact was made with the metal casing that was energized and employee was electrocuted.
