29 CFR 1910.146(c)(9)(i): Each contractor who was retained to perform permit space entry operations did not obtain any available information regarding permit space hazards and entry operations from the host employer: a) New York Avenue Apartments, 233 North New York Avenue, Atlantic City, NJ 08401: On or about August 15, 2014 the employer did not obtain available information regarding permit space hazards from the host employer. An employee died in an elevator shaft after being crushed by the elevator counterweight. NOTE: IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM. FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.
