29 CFR 1910.178(n)(4): Industrial truck driver(s) were not required to slow down and sound the horn at cross aisles and wherever vision was obstructed: A) Shipping Department. Runway #3, on or about July 24 , 2014, an employee operating a Hyster forklift traveled through a doorway (equipment doorway), where the operators vision was obstructed, without sounding the horn on the forklift or slowing down, exposing employees to a struck-by hazard.
