29 CFR 1915.152(b)(4): The employer failed to document an occupational hazard assessment which should have included: Occupations involved in the survey, the date in which the survey took place and the name of the person who performed the hazard assessment. F/V Alpine Cove, St Hermon Harbor, Kodiak, AK: On or about 6 June 2014 and times prior thereto, the employer did not document a hazard assessment for welding, among other hazard assessments on the vessel, prior to conducting hot work in the engine room on a R-22 Freon refrigeration system. This condition exposed employees to welding and asphyxiation hazards from a R-22 Freon leak on the vessel.
