29 CFR 1910.119(f)(3): The operating procedures were not reviewed as often as necessary to assure that they reflect current operating practice, including changes that result from changes in process chemicals, technology, and equipment, and changes to facilities: a) PC West 1205 Building: Operating procedure 60 was not updated to reflect the removal of the HFA monitors from the 1205 building, on or about 5/19/14. b) PC West 1205 Building: The description of the interlock action is not accurate in OP No. 96 SB 166 Interlock test procedures. The interlock does not block the C390 vent to SB 166, on or about 5/19/14. c) PC West 1205 Building: The description of the interlock action for 1822 PC was not accurate in Operating procedure 79, on or about 5/19/14. d) PC West 1205 Building: The operating procedures for some procedures in the area did not include some of the conditions under which emergency shutdown is required. NOTE: THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT CERTIFICATION FOR THIS ITEM. FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.
