29 CFR 1926.502(d)(17): The attachment point of the body belt was not located in the center of the wearer's back. The attachment point of the body harness was not located in the center of the wearer's back near shoulder level, or above the wearer's head. (a) House roof at 619 Towsend Place, Powder Springs, GA: An employee was exposed to fall hazards while using an Ascension positioning belt/mountain climbing with rope grab located at the wearer's waist on a combined 10:12 and 14:12 pitched roof at heights of approximately 14 feet from the eave to the lower level.
