29 CFR 1910.147(d)(4)(i): Lockout or tagout devices were not affixed to each energy isolating device by authorized employees: On or about December 17, 2013, and at times prior thereto, water blasting operations were performed on Coating Tank #2 without placing lockout devices to valves that controlled the flow of material and heat, exposing employees to the hazards of entrapment.
