29 CFR 1910.219(c)(4)(i): Unguarded projecting shaft end(s) did not present a smooth edge and end and projected more than one half the diameter of the shaft: On or about 10/30/13 at Augusta, GA: Employees were exposed to caught between hazards while performing work in the Printer/Former Area. The employer failed to ensure that projected shaft ends were at a length less that one half the diameter of the shaft in the following instances: a. Printer #1: The Lof & Sons Printer had 4 shafts that were 5 inches in length and 1 1/4 inches in diameter. b. Printer #2: The Lof & Sons Printer had 4 shafts that were 5.5 inches in length and 1 1/4 inches in diameter. c. Printer #9: The Lof & Sons Printer had 8 shafts that were 7 inches in length and 1 1/4 inches in diameter. d. Printer #10: The Lof & Sons Printer had 4 shafts that were 4.5 inches in length and 1 1/4 inches in diameter. e. Printer #11: The Lof & Sons Printer had 4 shafts that were 4.5 inches in length and 1 1/4 inches in diameter. f. Printer #14: The Lof & Sons Printer had 8 shafts that were 4.5 inches in length and 1 1/4 inches in diameter.
