29 CFR 1910.305(b)(1)(ii): Unused openings in boxes, cabinets, or fittings were not effectively closed: a) Paper machine #1, west side wall in the basement on or about February 5, 2014, the 480-volts electrical disconnect panel box had an unused opening on the right side from a missing knockout, exposing employees to electrical shock hazards. b) Paper Machine #1, inside the MCC Room - on or about February 05, 2014, a breaker panel had unused openings in the face plate, exposing employees to an electrical shock hazard. ABATEMENT CERTIFICATION IS NOT REQUIRED
