29 CFR 1910.147(c)(5)(i): Locks, tags, chains, wedges, key blocks, adapter pins, self-locking fasteners, or other hardware were not provided by the employer for isolating, securing, or blocking of machines or equipment from energy sources: a. On or about September 5, 2013, at the production area-Cosani line of the above addressed jobsite, an employee who performed servicing of equipment was not provided with hardware as necessary to lockout air valves, exposing the employee to a caught by hazard. b. On or about September 5, 2013, at the production area-PW33 Case packer of the Sinchro 4.0 of the above addressed jobsite, an employee who performed servicing of equipment was not provided with hardware as necessary to lockout air valves, exposing the employee to a caught by hazard.
